Thank you for choosing Universal Chiropractic! By booking online, you are making an appointment request. Staff will contact you within 24 hours to confirm or reschedule your appointment! In the event that you do not hear from one of our staff members before the time/ date you chose, your appointment is NOT confirmed and we ask that you call us before you come into the office.
If you aren't looking for any specific doctor, click around on each doctor to see if they have times that work for you.
If you are a new patient, there is an option to print and fill out your New Patient Paperwork so you are able to bring it in for your first appointment. Go to home page, click on the "More" tab, click "Online Forms," click "Download New Patient Forms." If you do not have these forms filled out before your first appointment, we ask if you please come in 25- 30 minutes early to your appointment so you can fill the paperwork out then.
